Men's Testosterone Health — Clinical Screening
ADAM Questionnaire
Androgen Deficiency in the Aging Male
The clinically validated 10-question screening tool for low testosterone — developed at Saint Louis University. Answer Yes or No to find out if androgen deficiency may be affecting your health, performance, and quality of life.
How the ADAM Questionnaire Works
Answer each of the 10 questions Yes or No based on your experience over the past month. Questions 1 and 7 carry extra clinical weight — a single Yes to either is enough to indicate a positive screen. The ADAM questionnaire was developed and validated at Saint Louis University and is used by physicians worldwide to identify men who may benefit from testosterone evaluation.

Frequently Asked Questions
Evidence-based answers about the ADAM questionnaire, low testosterone, and TRT at Body Symmetry MD.
The ADAM (Androgen Deficiency in the Aging Male) questionnaire is a validated 10-question clinical screening tool developed at Saint Louis University by Dr. John Morley. A positive screen occurs when a man answers Yes to question 1 (decreased libido) or question 7 (erectile difficulties), or Yes to any 3 or more of the 10 questions.
Common symptoms include decreased sex drive, low energy, reduced muscle strength and endurance, increased abdominal fat, depressed or irritable mood, difficulty achieving or maintaining erections, poor sleep quality, brain fog, decreased motivation, and declining physical and work performance.
The ADAM questionnaire screens positive if you answer Yes to question 1 or question 7, OR Yes to any 3 or more of the 10 questions. A positive screen warrants lab testing and physician evaluation — it is not a diagnosis.
Body Symmetry MD recommends a free consultation with one of our physicians. Your provider will order a comprehensive hormone panel including total testosterone, free testosterone, estradiol, LH, FSH, and SHBG. A personalized TRT plan is developed from these results.
Testosterone peaks in the late teens and declines roughly 1 percent per year after age 30. Clinically significant low testosterone is most common in men over 40 but can affect younger men due to obesity, chronic stress, or underlying conditions.
TRT under physician supervision has an established safety profile for men with clinical hypogonadism. Body Symmetry MD uses bio-identical testosterone customized to each patient's labs and symptoms, with ongoing monitoring. Most men report improvement in energy, libido, and mood within 4 to 8 weeks.
